Our knowledgeable Enrollment Coordinators assist in the education efforts on the Affordable Care Act, including group presentations in (English and Spanish) and community outreach within the community. These efforts will be within UMMA’s primarily locations including, Florence/Hoover, Fremont High School and the surrounding areas.

**Outreach and presentations are available upon request for the community.

What you’ll need to bring for enrollment:

1. Picture identification (eg California ID, Passport, Consular ID, School ID, Driver’s license)
2. Proof of Address (Mail no older than 60 days) (eg ID with correct address, utility bill addressed to applicant/parent, letter addressed to applicant)
3. Proof of Income (Pay stubs no older than 60 days) (eg 2017 Income Taxes, three most recent pay stubs, written self-affidavit of income)
4. Social Security Number (eg Social Security Card, 2017 Income Taxes)
5. Proof of Citizenship (Foreign-born citizens) (eg Certificate of Naturalization/Citizenship, Permanent Resident Card, Work Permit)
6. Date of Birth
